When you submit the appointment form on this site we collect your name, email, phone number, preferred date and time, and any reason for visit you choose to share. We do not collect payment information through this website.
How we use it
Your information is used only to confirm, reschedule, or follow up on the appointment you requested, and to provide care during your visit. We do not sell or share your information with third-party marketers.
How it's stored
Appointment submissions are stored in a private database protected by row-level security. Public visitors and signed-in users of this site cannot read, edit, or delete appointment records — only the practice can, through a passcode-protected admin area.
Your rights
You may ask us to access, correct, or delete the information you've submitted at any time by calling the office. We will respond within a reasonable time.
